Dzwonkiewicz of Schaumburg since 1968 will be held Saturday, May 14, 2016 1:00 - 3:00p.m. at the Countryside Funeral Home and Crematory 1640 Greenmeadows Blvd. (at Barrington Rd.) Streamwood/Schaumburg. Cremation was private at the Countryside Crematory.
Born November 29, 1940 in Chicago, Phyllis died Wednesday May 4, 2016.
She is the beloved wife of the late Carl; loving mother of Phillip (Jennifer) and Timothy (Tiffany); cherished grandmother of Joseph, Emma and A.J.; many loving nieces and nephews. Phyllis will also be missed by her devoted cats, Sahara and Boots. (630) 289-8054.
