Phyllis Jeanne Ptak, beloved wife of the late Ronald Sr.; loving mother of Ronald Ptak Jr., Brandon (Brian Hofacker) Ptak and Shellie (Jeremy Bockstahler) Ptak; dear grandmother of Caitlyn Rae, Mackenzie, Dylan and Cassidy; fond sister of Julie (Greg) Moloznik and the late Jack O'Brien; caring daughter of the late John P. and Loretta O'Brien.
Visitation Thursday 3:00pm until time of service 7:30pm at The Countryside Funeral Homes and Crematory, 333 South Roselle Rd., Roselle. Burial Friday will be private.
