Jose Leiva, age 66, loving husband of Madelym; beloved father of Lance, Joseph, Lisa and Richard; grandfather of Elizabeth and Michaela; dear son of Carmen Leyva; also survived by brothers; sisters; nieces; and nephews. Memorial service Friday 6 to 9 p.m. at Countryside Funeral Home, 1640 Green Meadows Blvd. (at Barrington Rd.) Streamwood-Schaumburg. Donations to the American Heart Association appreciated.
