Visitation for Joann R. Cebulski nee Matz of Huntley, formerly of Streamwood for 48 years will be held Friday, April 23, 2021 3:00p.m. until time of service 7:00p.m. at the Countryside Funeral Home and Crematory 1640 Greenmeadows Blvd. (at Barrington Rd.) Streamwood/Schaumburg. Following services, cremation will be private at the Countryside Crematory.

Joann is the beloved wife of the late Frank, Sr.; loving mother of Frank (Lori) and Deb (Dan) Leake; cherished grandmother of Brian (Jen) Casey, Christy (Jeff) Busam, Dan (fiance Brenna Sharp) and James (Belle Mareci) Leake; proud great grandmother of Emily, Jacob and "one on the way".; dear sister of Ruth (the late Joe) Zumpano; sister-in-law of Gerry (Myrna) Cebulski; aunt of many loving nieces and nephews.
