Gioconda "Jackie" L. Hornacek (nee Polifronio), 95, of Carol Stream died October 28, 2016. She was the beloved wife of Anthony J. Hornacek for 70 years; loving mother of Mary Anna (Terry) Scudieri, Antoinette (David) Wysopal and David J. (Georganne); proud grandmother of David Jr, Bryan, Terry Jr, Daniel and Stacey; cherished sister of Patricia (Donald) Gattone, Mary (the late Floyd) Hepworth and the late Rae Tricoci-Mancha. She will also be missed by her honorary daughter Leslie Pilkington.
Jackie enjoyed traveling; she was an amazing cook and seamstress. Most of all she loved her family, and she liked to say the word “Amen”.
Visitation Thursday November 3, 9:30a.m. until the time of Mass 10:30a.m. at Resurrection Catholic Church 30W350 Army Trail Road, Wayne, IL 60184. Entombment Queen of Heaven Cemetery. In lieu of flowers donations to Resurrection Catholic Church would be appreciated.
