Visitation and funeral services for Mr. Frank Polk of Hanover Park will be held Friday, July 22, 2011 from 3:00 p.m. until time of service at 8:00 p.m. at the Countryside Funeral Home 1640 South Greenmeadows Blvd. Streamwood/Schaumburg. (630-289-8054) Following the services, cremation will be private at Countryside Crematory.
