Daniel “Dan” Adam Mazzuca, 33, of Schaumburg, died Monday, November 23, 2009. Born July 19, 1976 in Park Ridge. Dan was the son of Carmen and Kathleen (nee Levandowski).
Dan enjoyed being outdoors especially when landscaping and fishing. He was also an avid sports fan who enjoyed soccer and watching the Cubs and the Bears.
Survivors include his parents; sisters Michelle (John) Lux and Melissa (Syril) Thomas; nieces and nephews Jessica Rose Lux and Elizabeth and Zack Thomas; and many uncles, aunts and cousins.
Visitation will be Monday, November 30, 2009 from 3 p.m. until time of service 7 p.m. at the Countryside Funeral Home, 1640 Greenmeadows Blvd, Streamwood. Following the service cremation will be private at the Countryside Crematory. Memorials to the Leukemia & Lymphoma Society, 651 W. Washington Blvd, Suite 400, Chicago, IL 60661 would be appreciated
