Born September 25, 1943 in Chicago, Jack died Sunday June 17, 2012.

He is the beloved husband of 43 years of Diana Lee nee Kutil; loving father of Bobbie Ann (Kenneth) DuMelle of McHenry and former father-in-law of David Zielinski, I and Sylvia Morales; cherished grandfather of Michael Morales, Crystal, Angela, Jaclene and Jack J. Lokcinski, David Zielinski, II and Hallee Cowsert; proud great grandfather of Briana Morales, Victoria Best and Jocelyn Muniz; dear brother of the late Patricia and Duane Miller; fond uncle of Patty Bledsoe, Bob, Johnny and Billy Miller.

Jack will be missed by many friends.
